I understand that all animals must be current on all vaccinations, laboratory tests and free of external and internal parasites or they will be treated at the owner's expense upon entry or discovery. If vaccinations or lab tests have been performed elsewhere I am required to provide written documentation upon admittance. All dogs must be currently on Heartworm Prevention and/or be current on heartworm testing. All cats must have been tested for Feline Leukemia and Feline Immunodeficiency Virus. All cats testing negative for Feline Leukemia must be currently immunized.

I understand you CANNOT guarantee the health of my pet. I understand and will not hold the clinic responsible for conditions that are unavoidable in boarding kennels, such as but not limited to weight loss, hair loss, upper respiratory infections, bronchitis and diarrhea. I understand that in the event of my pet’s illness, the staff will immediately try and contact me or my emergency agent.

In the event that contact is unable to be made, I authorize the doctors of Battlefield Animal Clinic and their medical staff to sedate my pet and/or perform such emergency procedures as deemed necessary for the health and welfare of my pet until myself or my agent can be reached. I agree to pay, in full, all charges for necessary services rendered for and to my pet. The clinic is to use all reasonable precaution against injury, escape, or the death of my pet.

The clinic and its staff will not be held liable for any problems that develop provided reasonable care and precautions are followed. I will call if my “pick-up” date changes so that you can plan accordingly. If I neglect to pick up my pet within 5 days of the date scheduled for discharge, and do not notify you within that time period, you may assume that the pet has been abandoned and are hereby authorized to dispose of the pet as you deem best and/or necessary.
